DOS scale
The Delirium Observation Screening Scale is a 13-item observational verbal and nonverbal behaviour scale. The observations can be done during regular care. To optimize the recognition of delirium, recording observations per shift is important.
When there is a change in behaviour and you are not sure whether it is dementia or a delirium due to a new underlying disease (like cystitis or pneumonia), record 3 times a day for 3 days.
